Application to the Data Controller

DATA SUBJECT APPLICATION FORM

This Application Form has been prepared in accordance with the Law on the Protection of Personal Data No. 6698, published in the Official Gazette dated April 7, 2016, numbered 29677 (hereinafter referred to as the “Law”) and the Communiqué on the Procedures and Principles of Application to the Data Controller, published in the Official Gazette dated March 10, 2018, numbered 30356. By means of this Application Form, you will be applying to Assos İlaç, Kimya, Gıda Ürünleri, Üretim ve Tic. A.Ş., acting in the capacity of data controller, within the scope of the right to apply granted to you under Article 11 of the Law.

In accordance with Article 13, paragraph 1 of the Law, you may submit your request to Assos İlaç via one of the following methods: 1. By sending a wet-signed copy to our address at Huzur Mahallesi Tosya Caddesi no:5 34773 Ümraniye, Istanbul,

2. By sending an e-mail to our registered electronic mail address assos@hs03.kep.tr with “Request for Information under the Law on the Protection of Personal Data” in the subject line,

3. By sending an e-mail to our e-mail address verikoruma@assospharma.com with “Request for Information under the Law on the Protection of Personal Data” in the subject line.

While the principle is that no fee shall be charged for the requests, if the process requires additional costs for Assos İlaç, a fee may be charged within the tariff determined by the Personal Data Protection Board.

We would like to remind you that additional verifications may be requested by Assos İlaç to confirm the accuracy of your identity information in the applications made. Personal information requested in this context is processed and used solely for the purpose of identity verification and fulfilling legal obligations under the Law.

In your written applications, the date your application is served to us as the data controller is deemed the application date; for applications made via other methods, the date your application reaches us as the data controller will be accepted as the application date. We kindly request that you attach any information and documents related to your request to your application.
